Home Health Process
Step 1 - Care begins with a physicians order. Patients, their family, or friends may request an order for home health from the physician. Discharge planners or social workers at hospitals and nursing and rehabilitation centers may also contact a patient's physician for an order for home health. Independent or assisted living communities wellness staff may also request an order on a patient's behalf.
Step 2 - Upon receipt Advent checks health records and insurance benefits. Then we assign staff and schedule the first visit.
Step 3 - Care begins. Upon the first visit a skilled professional performs an overall assessment to the patient. Then a comprehensive plan of care is established in coordination with the physician, patient, their family or associated caregivers. Scheduled care continues.
Step 4 - Changes, updates, and other important information is communicated through the entire process to the patient, their family or associated caregivers, and their physician.

Freedom of Choice
According to federal law, a patient has the right to choose the institution, agency, or individual to provide their home health care. When home health care is recommended, the patient should ask for their preferred provider.
Section 1802 of the Social Security Act "seeks to ensure that free choice is guaranteed to all Medicare Patients." The law states: "Any individual entitled to insurance benefits under this title [i.e., Medicare] may obtain health services from any institution, agency, or person qualified to participate under this title if such institution, agency, or person undertakes to provide him such services." This statement gives patients freedom to choose whom they want as their provider of post-hospital services.
